Thank you for scheduling your pet(s) for spay/neuter surgery. The date your pet is scheduled for is ____________________ . We need you bring your pet to the clinic between 7:30-8:30 am. You will be able to pick you dog up at 5:00 pm or your cat at 5:30 pm. When you take your pet home make sure he/she has a warm place to recoup, inside the house is preferred.

Take your pet‘s food away after 10 pm the night before (unless it is a puppy or kitten under 4 months old) this will help prevent him/her from getting sick when given the anesthesia. It is okay for them to have a small amount of water.

We require a $5.00 rabies vaccination if your pet is not current. Bring proof of Rabies vaccination (certificate) if your pet is current. We also offer the dog and cat core vaccinations at the time of surgery if you choose to get them. All fees should be paid at the time of check-in with cash or a money order.

When you arrive at the Clinic please come in and fill out the paperwork before bringing your pet(s) in. Dogs should be on a leash and cats in some type of secure carrier.

Volunteers for Animal Welfare supplements the cost of the surgeries therefore we need to see income verification showing you qualify for our services. A pay stub, W2, tax statement, or assistance card will work for proof.

Heartworm Test $15.00 (dogs) FIV/Leukemia Test $20.00 (cats)

E-Collar $5.00 / If your male dog likes to lick himself, he will need one!

Cat Cardboard Carrier $5.00 (All cats must leave in a carrier)
